This is the Template Editor page, circled below is the Preview button that allows us to view anytime how changes applied would look on the final filling form.

Then we have the Template's name and description.

Under it are the different fields.

Notice that the Full name is Required (mandatory to fill) and cannot be removed from a Client forn.

The other fields (Birth Date & Passport Number) can be removed using the trash bin icon, or be made non-mandatory by unchecking the Required field.

The arrows below mark the Add System Field and the Add Custom Field drop down menu.

Using the Add System Field allows us to scroll through Regulait's ready-to-use assortment of Client information fields we might need.

The 'Add Custom Field' drop down menu allows us to add our own field, be that a Text, Number or a Date field.

Add a custom field, name it, and it shall appear to the Client on the form.

Scrolling down the Edit page, we can add a Document Upload field, and name it to our choice.

We can decide how often Clients, being added through this Template, will be screened for Matches and potential risks.

And we can select the Matching Ratio.

In the example below, a Matching Ratio of 0.3 is set.

This means that any match above 0.3 will appear in the Matches list.

It requires careful verification to ensure the individual is not concealing their true client identity or, if it is the client, that it is safe to add them.

more Template editing options

This is an example of an edited Template, where the Full name, Middle name, Birth date, Passport number, and our own customized Zipcode fields are all mandatory to fill.

While the Country of Birth field is optional.

Let's click on Preview to see how the final form would look.

Now look below at the different fields we've added.

The Required fields are mandatory for using blood donations, while the Ethnicity field is optional, as it might help doctors for research purposes, but is not relevant for using the donations.
